Responsibility
- Conduct comprehensive medication therapy management (MTM) reviews for patients with chronic conditions.
- Provide expert pharmaceutical care and counseling to patients and healthcare professionals.
- Monitor patient health records to identify potential drug interactions and adverse effects.
- Collaborate with physicians and nurses to optimize treatment plans and ensure medication safety.
- Maintain accurate patient records and ensure strict adherence to HIPAA and regulatory compliance.
- Manage inventory and assist in the procurement of pharmaceutical supplies.
Qualification
- Master’s degree in Pharmacy (Pharm.D.) or equivalent qualification.
- Current New York State Pharmacist License in good standing.
- Strong knowledge of pharmacology, drug interactions, and therapeutic guidelines.
- Excellent verbal and written communication skills.
- Detail-oriented with the ability to manage multiple priorities in a fast-paced environment.
- CPR/BLS certification preferred.
