Responsibility
- Review and verify prescriptions for accuracy, dosage, and potential drug interactions.
- Provide comprehensive medication counseling to patients and caregivers regarding usage and side effects.
- Lead Medication Therapy Management (MTM) programs to optimize chronic disease management.
- Collaborate closely with physicians and care teams to ensure seamless clinical coordination.
- Manage pharmacy inventory, including procurement, stocking, and controlling drug supplies.
- Ensure strict adherence to legal, ethical, and professional standards of pharmacy practice.
- Maintain precise patient records and documentation in full compliance with HIPAA regulations.
Qualification
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.
- Valid New York State Pharmacist License (NYS) in good standing.
- In-depth knowledge of pharmacology, drug interactions, and therapeutic guidelines.
- Strong verbal and written communication skills with a focus on patient education.
- Ability to thrive in a fast-paced, high-volume clinical environment.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Proficiency with pharmacy management systems (e.g., Rx30, McKesson).
