Responsibility

  • Deliver comprehensive, patient-centered primary care services to a diverse patient panel
  • Diagnose and treat acute and chronic illnesses, injuries, and health conditions
  • Order, perform, and interpret diagnostic tests and procedures
  • Develop and implement personalized treatment plans in collaboration with patients
  • Coordinate care with specialists, hospitals, and ancillary services
  • Maintain accurate and timely electronic health records (EHR) documentation
  • Provide preventive care, health education, and counseling to patients
  • Participate in quality improvement initiatives and clinical performance metrics

Qualification

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited school of medicine
  • Successful completion of an ACGME-accredited residency program in Family Medicine or Internal Medicine
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine
  • Active, unrestricted New York State Medical License and DEA registration
  • Current BLS and ACLS certification
  • Excellent communication, interpersonal, and patient engagement skills
  • Proficiency with EHR systems (preferably Epic or eClinicalWorks)
  • Commitment to providing culturally competent, high-value care
