Responsibility
- Deliver comprehensive, patient-centered primary care services to a diverse patient panel
- Diagnose and treat acute and chronic illnesses, injuries, and health conditions
- Order, perform, and interpret diagnostic tests and procedures
- Develop and implement personalized treatment plans in collaboration with patients
- Coordinate care with specialists, hospitals, and ancillary services
- Maintain accurate and timely electronic health records (EHR) documentation
- Provide preventive care, health education, and counseling to patients
- Participate in quality improvement initiatives and clinical performance metrics
Qualification
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited school of medicine
- Successful completion of an ACGME-accredited residency program in Family Medicine or Internal Medicine
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted New York State Medical License and DEA registration
- Current BLS and ACLS certification
- Excellent communication, interpersonal, and patient engagement skills
- Proficiency with EHR systems (preferably Epic or eClinicalWorks)
- Commitment to providing culturally competent, high-value care
