Responsibility
- Provide comprehensive medication therapy management for complex patients
- Collaborate with physicians on clinical protocols and drug formulary decisions
- Conduct medication reconciliation and safety audits
- Lead patient education programs on chronic disease management
- Mentor pharmacy interns and junior staff
- Participate in hospital committee meetings (e.g., Pharmacy & Therapeutics)
- Utilize EMR systems for clinical documentation and reporting
Qualification
- Doctor of Pharmacy (PharmD) degree from ACPE-accredited institution
- Current Illinois pharmacist license with no restrictions
- 3+ years of hospital or clinical pharmacy experience
- BCCCP or Board Certified Pharmacotherapy Specialist credential preferred
- Expertise in anticoagulation, oncology, or cardiology pharmacy
- Advanced knowledge of FDA regulations and JCAHO standards
- Strong proficiency in Epic EMR and clinical decision support tools
