Responsibility
- Manage complex medication therapy management for chronic disease patients
- Conduct comprehensive medication reviews and clinical interventions
- Lead pharmacy-based vaccination and health screening programs
- Mentor pharmacy technicians and interns through structured training
- Implement evidence-based clinical protocols and quality improvement initiatives
- Collaborate with healthcare providers on medication reconciliation
- Utilize dispensing technology and automated inventory systems
- Participate in hospital committee meetings and policy development
Qualification
- Doctor of Pharmacy (PharmD) from ACPE-accredited institution
- Active Illinois pharmacist license with no disciplinary actions
- 3+ years progressive clinical pharmacy experience
- Board certification in Pharmacotherapy (BCPS) preferred
- Strong proficiency in electronic health records (EHR) systems
- Demonstrated experience in medication safety protocols
- Excellent communication and patient counseling abilities
- Valid CPR certification
