Responsibility
- Dispense medications with precision and provide personalized patient counseling on usage, interactions, and side effects
- Lead clinical initiatives including medication therapy management and chronic disease state management
- Supervise pharmacy technicians and ensure compliance with TPA and provincial regulations
- Collaborate with healthcare providers to optimize patient outcomes through interdisciplinary care
- Manage inventory systems and implement cost-effective pharmaceutical supply chain solutions
- Conduct health screenings and immunization services to enhance community wellness
- Maintain accurate patient records and ensure HIPAA-compliant documentation practices
Qualification
- Doctor of Pharmacy (PharmD) degree from an accredited Canadian institution
- Active Ontario College of Pharmacists (OCP) license in good standing
- Minimum 5 years of progressive pharmacy experience with leadership exposure
- Certification in Medication Therapy Management (MTM) preferred
- Expertise in electronic health records and pharmacy management software
- Strong knowledge of compounding, sterile preparation, and IV admixture
- Demonstrated commitment to evidence-based pharmaceutical practices
- Exceptional communication skills with multilingual capabilities considered
