Responsibility
- Manage complex medication therapy management for high-risk patients with chronic conditions
- Lead clinical pharmacy initiatives including anticoagulation management and diabetes education
- Collaborate with multidisciplinary teams to optimize patient outcomes and reduce hospital readmissions
- Supervise pharmacy technicians and provide mentorship for junior staff
- Implement and maintain compliance with JCAHO and state board regulations
- Conduct medication safety audits and develop process improvement protocols
- Participate in formulary management and procurement decisions
Qualification
- Doctor of Pharmacy (PharmD) degree from ACPE-accredited institution
- Active Illinois pharmacist license with at least 5 years of clinical experience
- Board certification in Pharmacotherapy (BCPS) or Ambulatory Care (BCACP)
- Experience with EMR systems (Epic preferred) and automated dispensing technology
- Strong leadership skills with demonstrated team management experience
- Excellent communication abilities for patient education and interdisciplinary collaboration
- Proven track record in medication safety and regulatory compliance
